About the Role:
Paragon Home Care is a non-medical home care agency serving older adults throughout Northern Virginia. We are seeking an experienced Registered Nurse (RN) to conduct in-home client assessments and develop individualized plans of care for our caregiving team. This is not a hands-on or bedside nursing position. Visits primarily involve assessing the client's needs, home environment, and safety. Most assessments take approximately 60 minutes, plus documentation.


Responsibilities:
  • Conduct comprehensive per-visit nursing assessments including ADLs/IADLs, cognition, mobility, fall risk, home safety, medication self-administration, and dietary needs
  • Develop, review, and update individualized plans of care that our caregivers will follow
  • Develop, review, and update individualized care plans based on assessment findings
  • Complete visits at required intervals and reassess clients when their needs or condition change
  • Document assessments and care plans in AxisCare (training provided)
  • Communicate relevant client needs or changes with our office team

Schedule & Pay:
  • Set your own schedule and choose which assessments you accept.
  • Visits are typically completed on weekdays during business hours.
  • Flat fee per completed assessment.
  • Independent Contractor (1099) position.
Requirements:
  • Active Registered Nurse (RN) license in the state of Virginia
  • Prior experience assessing older adults: home health, home care, or community nursing preferred
  • Strong clinical assessment and critical thinking skills
  • Excellent documentation skills
  • Reliable transportation, valid driver's license, current auto insurance
  • Ability to work independently with minimal supervision
  • Strong interpersonal and communication skills with patients, families, and care teams
  • Able to clear a Virginia criminal background check (required by state law for home care organizations) 
  • Must provide TB screening documentation
